I Wish I Knew: Make a list of physicians.

Everyone has a team of physicians but do your family members know who they are? Would they know how to reach them in a time of need? The answer is no, unless you tell them. The solution is simple, make a list of your dream team of doctors and share it with your family.

Include your physician’s name, office address and contact information on your master list. If you access your medical records via an online portal, provide the portal web address, username and password to access it. Include all of your physicians including primary care, gynecologist/obstetrician, ophthalmologist/optometrist, cardiologist, neurologist, allergist, dermatologist, endocrinologist, gastroenterologist, oncologist, physiatrist, psychologist, urologist etc. Don’t forget your dentist, endodontist, periodontist, orthodontist etc.

And while you are making your list, consider including a copy of your medical, dental and prescription insurance cards with the information. The name and address of your pharmacy or online pharmacy is beneficial too along with a list of drugs which are on automatic refill. Note your preferred hospital choice if you have one.

Don’t make your family have to guess who your physicians are. Help them navigate this important part of your life with a master list of physicians. Your family will thank you for putting this together for them!
